Empire Airlines in the early 1980s. The aircraft was a Fokker 28. It was also the shortest route I have every flown, Utica, NY to Syracuse, NY. Time was under 10 minutes. The cabin crew even served apples during the short flight.

 

The full flight route for that day that started with Empire Airlines was:

 

Empire: KUCA-KSYR-KJFK F28

Forgot one - you can add the following to my list:

 

-- Allegheny Commuter (I think the actual operator was Suburban Airlines, but I'm not sure), round trip from Pittsburgh to State College, PA in July 1984.  Equipment was a Shorts 360.
