Assuming you installed latest package available from website, try replacing "mce.exe" with this one.

I you get an instance where your speech is not being recognized after trying a couple of times, press and hold PTT switch for 2 seconds, then release it.

It could be that it's stuck in ATC mode, where all commands for interaction with FO are suspended and those for ATC are enabled.

I also have this same problem. What is the PTT switch/button?

There is still the possibility speech reco goes occasionally missing.

Every time you hold PTT down, MCE has to suppress speech commands for interaction with Fo, checklist and other custom commands (Voxscript and Voxkey).

It then enables ATC specific commands.

Ideally, the process is reversed smoothly every time you release it.

On the occasional letdown (no response from FO after a couple of tries), press and hold PTT for 2 or 3 seconds, then release it.

That should ensure MCE comes out of ATC mode.

For Vatsim, now that you have setup your PTT, go to "General" tab in MCE UI and click <More options> button.

In "Advanced Options" panel, click <View Config> button.

"mce.ini" will open in Notepad.

Go to {GAMEPAD] section and add this line.

UseWithOtherAtc=1

Could eventually, just change "useWithVoxAtc=0" to "useWithVoxAtc=1"

Either option will do.

Save thee file, then disable <Talk to ATC controllers> feature in MCE panel and restart it.

From then on, whenever MCE sees that joystick button (PTT) down, it assumes you're talking to some other unknown ATC system.

Same will happen when you hold the <END> key down.

Speech reco resumes when you release PTT.

By default, it's the <END> keyboard key for Vatsim use.

See my other reply above on how to use joystick as well.
