Disable full screen optimization or enable triple buffering with vsync.

and... update your graphics driver.

What type of graphics card do you have?  In fact, what are your system specs?

Enabling triple buffering with vsync seems has resolved this issue. Thanks you very much and best wishes for Christmas.

Perform a DDU graphics driver wipe, then update to the latest graphics driver, and before starting P3D I would clear your P3D Shaders and I would also delete your Prepar3d.cfg file.

Still seems to be fps variance beyond the refresh, as simbol mentions not VSyncing and that would mean selecting:

Unlimited + Vsync=On + TB=On

I would also go into the NVidia Control Panel and force a restore apply on the 3d pages main and P3D profiles, even if means to have to make any setting first to get the Restore button. That can clear some odd issues.

Another thing to check is to set 20 fps on the slider, try VSync=Off with that.
