Strange SB4 Problem

Had to reinstall Win&, FSX and all other add-ons due to a driver problem.  Did a clean start all over again.  Encountered a strange problem.. Squawkbox 4 does is not playing the audio on events.  As a result of which events such as atc refresh, private message from the controller, etc go unnoticed.

 

When I pull down the menu in the SB4 Options - Display - Sound section, I can see all the options and if I select any one of them, they show up and they also play when the play key is pressed.  However, the sounds don't play after I exit out of the options and go in to normally using SB4.

 

The restore defaults option seems to be doing nothing either.
