March 17, 20224 yr Our ATSB here in Australia released the safety report today relating to the Malaysian Airlines A330 that departed YBBN with covers installed on all three pitot tubes back in 2018. It goes into great detail analysing the procedural shortcomings that resulted in the covers not being removed but to me by far the most interesting aspects were an in depth discussion of the airline's cockpit take off procedures, the delegation of duties between PIC and FO and the in-flight deployment of the BUSS system (backup speed scale).
