using STAR's and approaches

I am not that familiar with the use of the above procedures as far as PIC 767 goes so i need some help with how to properly enter them into the FMC.. are u supposed to use both the STAR (where available) and the corresponding runway approach together or do u only use one or the other? I tried using both of them for a flight into JFK. can't remeber the exact STAR i used but i did use the RW4L approach and i got a flight route which did not seem right (flew right past the airport to a waypoint about 100 miles over the atlantic and then turned 180 to go back to the approach to 4L. is this really how these guys fly in the real world ? any help/tips on the correct of STAR's and approaches would be greatly appreciated. cheers

Flight planning is an art and the use of SID's and STAR's takes some practise. And having actual paper charts alleviates much of the confusion that your STAR & APP led to.
