Are they know cities like honolulu in the free download? If so How do you position yourself there

Only Hawaii. The other islands are very low rez and have no detail, plus you need to FLY there to see for yourself. Trust me. Unless you have the DLC, its not worth the trip.

In the free download (absent any DLC) you have Hilo, which is the county seat of the island of Hawai'i. It is on the east side of the island and served by Hilo Intl. On the west side you have Kailua-Kona and Kona Intl. Sitting up above those two at around 2700' is Waimea and it has Waimea-Kohala Airport and at the northern tip of the Big Island is Hawi, which is served by Upolu Airport.While The Big Island is larger than all the other islands combined, it only has about 10% of the State's population. Around 100,000 if I recall. There are many small communities scattered all over The Big Island. And many small airstrips to hunt down and conquer.
