I noticed that "cabin smoke" is not an option on the "Failures" menu.  (correct me if I'm wrong)

 

Is there a failure chain that will trigger it instead, like leaving a cargo fire going too long?  I'm trying this on the freighter model if it matters.

You are correct for both the passenger and freighter versions.

There are no main deck fire extinguishing/detection system on the main deck of the freighter. You can still use the cabin smoke checklist even if there is no smoke warning.
