I turned off vsync & triple buffering in P3D but turned on 1/2 vsync & triple buffering in the Nvidia panel.  Also turned FXAA on in Nvidia and off in P3D and I think I am getting better results.  I'd love to know how others have theirs configured and if this makes sense?
