Greetings, new to this, but after you setup Axes and Ohs for a PMDG 737 or any other aircraft, do you have to run it everytime for the aircraft to do what you set up?

In other words run it at the same time all sessions of flight sim?

Yes. AAO is a stand alone program. It has nothing whatsoever to do with the controller assignments in the simulator, it is an entirely separate thing. It can only do its job when it is actually running.
