Yes, download the new installer and you are fine...

And uninstall any previous versions via the control panel before installing into v4!!

If your question is specifically asking about the PMDG 777-300ER if you do not already have it, then, yes you have to buy it first, on top of the Base 777-200 LR/F, and yes, as Marc has pointed out , it will work in P3Dv4.
