During my testing, i always rolled back the AP to pitch mode, before setting a new QNH, and then going back to VS and Armed Target mode. Yes, there is a long-standing problem in the sim. I know it affects the Baron, and maybe other aircraft as well. If you sync the altimeter to current QNH with the B key, the pressure correction will be correctly applied. But, if you manually adjust the QNH with the setting knob, the correction will not be applied. This is not a problem in the US - you just sync the altimeter with the B key as often as necessary until passing transition altitude of 18,000 feet at which point pressing B will set standard pressure of 29.92/1013. It is a problem in Europe or other places where the transition altitude is much lower. MSFS uses 18,000 feet for the transition altitude everywhere.
