Yes, there is a long-standing problem in the sim. I know it affects the Baron, and maybe other aircraft as well. If you sync the altimeter to current QNH with the B key, the pressure correction will be correctly applied. But, if you manually adjust the QNH with the setting knob, the correction will not be applied. This is not a problem in the US - you just sync the altimeter with the B key as often as necessary until passing transition altitude of 18,000 feet at which point pressing B will set standard pressure of 29.92/1013.

It is a problem in Europe or other places where the transition altitude is much lower. MSFS uses 18,000 feet for the transition altitude everywhere.

The bug is in the manual setting of barometric pressure.
