Probably because you have multiple audio devices and either MCE is listening on the wrong device or FO speech output is on the wrong one. If using a conventional headset, plug it to the back of the PC (especially with Realtek audio) Go to <About> tab and check which devices are listed under <Sound In> and <Sound Out> If either is incorrect, run "assign sound device" tool Windows Start->Multi Crew EXperience->Assign sound device Carefully select devices then click OK and let the tool make automatic adjustments if needed.
