I'm experiencing an issue with a flight plan from CYOW to CYYZ. The TOC is somewhat after the first STAR waypoint. I import the plan from simbrief successfully. It shows the cruise altitude of FL380 which actually shows up as the planned altitude at the TOD. The problem I'm experiencing is that the ATC stops giving climb instructions before issuing the one for the TOC altitude.

I checked the log and I can see the rOnReachedNextClimbAltitude events followed by the NextAssignedAltitude indications for lower altitudes. When I reach the first STAR waypoint I see a rOnFirstSTARWaypointReached followed by "State Changed to: STARDescent" and the descent instructions. Then I get no more rOnReachedNextClimbAltitude events and get no more climb instructions.

I haven't flown this route in a while but I could swear that it worked for me before. I end up, in this case, just climbing to the TOC without instruction.

You need to make sure your TOC is before the first waypoint.  The program assumes that once you reach the STAR first waypoint, it's time to start descending.  

Sounds like you could just lower your cruise altitude or increase your climb rate and it should work fine.
