Help with takeoff

Hi, I occasionally have trouble with takeoff and I'm wondering if I'm doing/saying the wrong things. Can you help me, what am I supposed to say when I get to the runway and I want clearance to takeoff? The typical problem I get (not every time) goes a little something like this:

Me: <my callsign> ready for taxi

ATC: <taxi instructions> hold short at runway 12L

(I get to the end of runway 12L and stop at the thick line)

Me: <my callsign> holding short at runway 12L

ATC: <my callsign> contact tower on <frequency>

(co-pilot changes frequency)

Me: <my callsign> holding short at runway 12L or <my callsign> ready for departure at runway 12L

ATC: <my callsign> contact ground on <frequency> for taxi instructions

 

... then repeat the dance ad infinitum until I give up and just take off without clearance.

2nd question: let's say I did just takeoff out of frustration like the above without clearance and breaking the normal sequence, is there something I can say just after takeoff to get my co-pilot to switch to the correct frequency for my ongoing flight so that I can continue as normal? If not, how do I know what frequency to switch to?

N.B. MSFS user, and I have my co-pilot set to switch frequencies for me.

You'll get sent back to ground for taxi when you tell tower you are not close enough to the expected departure position.  This can happen if the runway is not defined properly in TaxiMnt or if you are not close enough to the end of the taxi route specified by the Taxi instructions.

If you press the Taxi button to the right of the map, then a blue line will show the assigned route and expected end point.  If the Taxi route is not specified because of lacing Taxi information for the specific airport, it would not appear.  In that case, you can look at the airport in Taxi Mnt and go to the intersection closest to the end of the runway.

On the second question, you can switch to the Departure Frequency you were given in the IFR clearance.  Then, after takeoff, do the same procedure you would do to start the program in mid-flight:

1.    Disconnect  and reconnect to the SIM
2.    File the flight plan
3.    Make initial call to ATC.  For Departure, use “<CallSign> climbing to xxxx feet” where xxxx is your initial assigned altitude from the clearance

ATC will give you a squawk code and all should proceed normally after that.

Thank you Dave. Given I am using VR, using the "maxi mnt" is a bit tricky. Anything you can do/suggest for VR users that don't have access to the P2A window or controls?

The only thing I can think of is to use TaxiMnt at the airport you're flying out of as part of your pre-flight planning to calculate the expected taxi route from your gate to the runway so you can be sure and use the expected intersection.

Also, if you are at the intersection of the extended runway/ overrun and get sent back to Ground, pull onto the runway and taxi up to the numbers and tell Tower you're ready for departure again.
