A few times, recently, ATC has directed me taxi to and hold midway at a runway. In the image, I wanted to, and was granted taxing permission to, taxi to R28, via B, D and hold short of runway 28/10 - whatever the instruction precisely was.

Do I automagically have permission to backtrack along in the 100 direction or not? If not, how do I gain that permission?

If the departure runway is 28, then when you get takeoff clearance, you'll be cleared to backtrack and takeoff runway 28.
