The Wilco pilot's guide shows the old location from the PIC version.

Not sure, but there is a button you can map for it in the fsx controls menu I think.

You can press Control+Shift+G to trigger TOGA, but, if you want to do it properly, the switches in the Wilco EVO 737 are cunningly located where they are on the real thing, i.e. two little switches under the throttle handles. Note that as per the real thing, you only have to press either button, not both of them to have TOGA trigger. Don't forget to arm autothrottle and engage the autopilot, or they won't trip.
