How do you get it?

First you need an aircraft that simulates decision height (all of the high end payware do). Then you need to set the DH in the FMC or MCDU.

I don't think default aircraft have it. Add on planes do.

There's a freeware addon gauge for that. Just google "Altitude Callout for FSX".
