OK so I finally managed to get it back to normal post patch 6. Switched of VSync OFF and restarted. Switched it back on and now I am back to normal.

The  in game VSync is notoriously flakey.

You are better off limiting the frame rate with your Nvidia software instead and disabling Vsync in the game.
