I really need your help. |Monitor losing signal and GPU fans

So I had this pc for about 2 years and for the past 5 months it has been giving me problems when playing games with high graphic settings.
Here is the problem description:
The monitor would lose signal, and GPU fans would spin at their max speed. The pc does not turn off, I can still hear people talking on Discord but can't do anything which forced me to fully shut down the pc by holding the power button.
I thought this was an issue with PSU so I ordered a new one but the issue still occurred.
My PC specs are as follows:
-GPU: Asus Strix vega56
-CPU: Ryzen 3600 non-x
-MB: Tomahawk b450 non-x
-Ram: 16gb corsair (2x8) 3200mhz
-PSU: Seasonic focus px-750 platinum
-SSD: 250gb kingstone
-HDD: 1tb WD
I really have no idea what to do and at this point, I'm hopeless. I would appreciate any help.
P.S Sometimes when alt-tabbing out of a game also triggers the issue

After recovering from the seizure reading your white text on black background, my gut feeling is that your GPU has an issue.

Is it seated properly?

What is the temperature of the GPU?

Maybe reinstall the the driver?

I personally would focus on the GPU..... 

Most of what is said on the Internet may be the same thing they shovel on the regular basis at the local barn.

Hahahahah I'm really sorry, I had dark mode on so I didn't really see how my text was.
-Yes the gpu is seated properly and both the PCI-e cables are fully plugged in
- The temp is around 65c-68c when playing games and 40c idle
- I have done that but to no avail...
Yea since now I know that my psu is not the issue I will just focus on troubleshooting the gpu as you have suggested. Thanks a lot I really appreciate the help and sorry again about the text format

If the driver didn't fix the issue I would be tempted to reinstall windows. Another thing to check would be a new monitor cable.
