It's happened to me before but only when I initiated a Go Around, I don't know if its a PMDG glitch or an FSX glitch, if it happened every single time, the only thing I can recommend is to maybe uninstall PMDG 747-400 then re-installing. I don't know what causes it.Sorry i just noticed that its the 747-8, I also have the 8 and still this has only happened to me like once or twice, and isn't something that happens often, once again maybe uninstall then reinstall.

I have had this happen at random times with different aircraft. I would not reinstall the PMDG based on this...it's an FSX issue.

Have'd the same and I'm sure it's not PMDG - it's FSX issure..

I can confirm this as well I have seen this in the Level-D but never the 747-8. It would be nice if this issue could be figured out.
