All you have to know is how to navigate via VORs and NDBs, and how to manually calculate a descent path, which is actually easier than learning how to use an FMS. In fact, there are some approaches where real world airline pilots have to know how to navigate via VORs and NDBs and have to know how to manually manage their descent. Take Kai Tak RW13 IGS approach for example. Intercept CH VOR (112.30) @ 8000 ft, descend and mantain 6000 and follow radial of 270° until 7 NM west of CH, then descend and maintain 4500, intecepting the SL NDB. Finally, you follow the RW13 IGS at 088° degrees, following it until the checkerboard turn, then you go full visual on the rest of the approach. It is quite frustrating flying this approach with the PMDG 747 because it does not recognize the IGS in the NAD/RAD page on the FMC.
