I found out that when using a runway extension you need something like this to fill in the discontinuity:

RW27060/22 (Fills in Disc)

RW27

 

Now how do you program that for another airport (like Canouan, TVSC)? What do the numbers mean? (Spec. 060 and 22)

I can only comment on your last question. What you have here is a bearing distance waypoint

 

RWY27 - FIX

060 - RADIAL FROM FIX

/22 - DISTANCE FROM FIX

If you are doing this to set up for visual approach, let the fmc do the work - on ARR page, do not select an instrument procedure, just runway number. You can then select how long do you want your final to be, and, if required, descend angle.
