The world.  You can eliminate the areas you do not want but there's no need to go to that length as FSX will only load the areas within around 100 miles of your aircraft.  Even if that area was set to 300 or 500 miles, it still would not have an affect on your sim.

So does that mean I can leave all my Orbx scenry checked in the USA if I'm flying in Europe?

Yes, it does.

Is that also hold true for photoreal scenery. I believe it loads all photo scenery no matter what, could be wrong.

According to Tabs over at PMDG, all photo scenery will load - http://support.precisionmanuals.com/kb/a108/vas-management-stopping-out-of-memory-oom-errors.aspx (look down to the photo scenery section). I had discovered it earlier as I use Process Monitor on occasion, set to monitor only FSX and FSX addons (as shown in the AVSIM CTD Guide), to see what was loading that should not be loading. You definitely do not want to have your photo scenery loaded as it will be taking up resources even if you are not using it.
