In the options menu, after selecting the Ultra preset (or whatever preset you wish to use), make one small change in any of the sub-settings. Something that will not have much of an effect (I turn off motion blur, I hate that). The preset should now show Custom. From what I understand, if the preset shows Custom, then the settings will stick. Including settings made by manually editing the UserCfg.opt file. Every time i open the sim i have to re do the custom settings again after first applied low settings and then going ultra then custom. it doesnt stick
