Your mileage may vary but I had lots of issues with DX12 and since I'm back on DX11 the sim runs better than ever.

You are still hugely CPU limited with such a low GPU usage. I would invest this GPU overhead in better image quality (by increasing render scale for example) as it is a known fact that if you move more load over to the GPU and reduce CPU limitation, you sim will run smoother. 

I agree to a point but right now I'm just attempting to be able to fly at all.  When I've increased render scaling in the past the performance tanked - when I reduced it to about 100 things were better.  I'll try boosting Render Scaling again and see what happens - it could be that with the other adjustments I've made I'll get better results.

As far as DX12 is concerned, one thing at a time.  If everything improves and I get continuous smooth flying again I'll leave it be,  If I get large variances in performance again I'll drop back to DX11 and test it out.

I'll be replacing the thermal paste tomorrow (I have it in-hand but need some 91% Isopropyl),

Updating:

After getting CPU temps in the 95° area (in flight) I finally refreshed my thermal paste.  The fan noise has backed off quite a bit and it's running about 84°.

I'm running Render scaling at 140 for now.  Everything is running smoothly - no more stuttering.  I'm getting 19-27fps in cloudy weather/city-scapes and 22-30fps with more open landscapes and moderate clouds.  I did try increasing scaling up to 200 - there was still room left for the CPU, BUT fps dropped.  I was getting tearing with VSync off so I turned it back on - no drop in performance.

That's a meaningful drop in temps, glad you got control of the problem you were having.  Though you're no longer thermal throttling, a consistent 85c is still rather hot IMHO.  My computer usually runs MSFS in the mid 60s, with infrequent and short lived excursions into the low-mid 80s.  Bottom line is if you're back to where you were, Happy Contrails!

I thought I might get more of a temperature drop, though it could have been the weather.  If I get a clearer day I'll take another measurement.
