COM1 and COM2 simultaneous use

The knobs with the arrows control the volume for the different audio channels, see this picture:

Simply turn both VHF-1 and VHF-2 up to listen to both channels simultaneously. You can tune the VHF-2 channel on the first officers audio panel.

Now does that have to look like that on both com1 and com2 or just com1?

The VHF-1 controls com-1 and VHF-2 controls com 2. If you mean whether you have to do it on both audio panels, I'm not sure if they're synced in PMDG's plane, but you should only need to do it on the left audio panel since that controls the captain's audio.
