Am I right in thinking that using 

Vertical Sync=Half Refresh Rate

And locking at 30 in ndivia Inspector 

And then clicking on 

'disable full screen optimisation' on the P3Dv4 program properties conflicts and the vsync don't work in fullscreen???

As best I know, this does not work in P3D.

Instead, try using RTSS as you were before..
