I had an incident rather then a death last night but still very embarrasing. Just took off from Madeira (LPMA) RWY05 on route to Cardiff (EGFF) when a strange gong went off and master caution above 10000 feet, never mind ignore it, it might go away. Hmm, master caution again and a failure on the cdu menu, PAX oxygen masks dropped, ooops and i just noticed the AFT DOOR OPEN light on the overhead. Thats no problem, i'll just shut the door, guess what, you can't shut the door in flight. Last option, emergency descent into Porto Santo (LPPS) good job nobody on duty in the control tower to have to explain stupidity! Ceri
