What are the psychological factors?  Once you've gone beyond aesthetically pleasing to what is basically a caricature, what drives someone to do this?  It's like women who starve themselves to the point of looking skeletal, but still think they're overweight.

I remember seeing pics of Arnold Schwarzenegger and thinking he looked good.  Generally well proportioned and not grotesque. 

 

When we're talking about very extreme muscle mass, then its usually the competitive aspect, bodybuilding is a sport. For somebody that's not a competitive bodybuilder to push themselves to the mass monster level with unhealthy eating practices, anabolic steroids, growth hormone and other drugs, then yes, in my opinion there could certainly be psychological factors at play. In terms of the natural bodybuilder, who's not prepared to risk their health with drugs, it's about health and fitness and simply having a more aesthetically pleasing body. 

Arnold had great genetics, he gained mass but still retained that aesthetically pleasing look. That was back in the golden age of bodybuilding. They took steroids back then, but not to the crazy extreme that people do now.

Not mass monster level, no, as such a thing would have been impossible without modern anabolic androgenic drugs, so they wouldn't have had a good reference for what such a body would appear like. 

 

Back when Arnold was competing, it was Frank Zane that was regarded as the most aesthetic.
