Always smoothness over IQ. Any stuttering ruins the immersion for me.

Smoothness over image quality all the way. Stutter destroys the experience big time.

"My" take on G-Sync: Good to prevent tearing so you can turn VSync off but then if using Track IR you need a steady FPS on it's own poll rate (120 or division of it, 60, 30) or there is micro stutter to be seen on fast moving terrain (less a problem in MSFS). So either you need to cap FPS exact at one of the poll rates or use VSync. Since my monitor has a 144Hz refresh it does not match the TrackIR poll rate so i had to lower it to 120Hz. For me the extra cost for the G-Sync was a waste of money.

Yes generally I am happy with it. It is not a silver bullet though.

I thought I would buy this and leave the refresh rate to 144Hz and frame rates in MSFS uncapped and all would be silky smooth.
 

I find with fast fluctuating frame rates it doesn’t quite keep up. The will be a slight stutter or 2 before it smooths out again. However I mostly mitigate this by using Riva Tuner and capping the frame rate at 40fps. This lessens the rapid fluctuations  and mostly eliminates stuttering.

I can see a big difference between 30fps and 40fps. Above 40fps and the returns for me diminish quickly. So this allows me to achieve my goal of 40fps but takes care of the smoothness when I drop below.

As an example flying the CRJ into JetStream Designs LFPO, I can hold 40fps most of the approach but as I get closer to the airport the frame rate will drop down to around 35fps. What I like is the the g-sync allows this to happen smoothly and seamlessly.
 

Another area I find a benefit is that in taxiing around some airports the frame rate can drop of considerably. G-sync just takes care of this and keeps a smooth experience.

Also the benefit of a g-sync capable monitor is the it still functions consistory at lower frame rates. I have been flying through cloud with a high Render Scale and even though the frame rate had dropped to 17fps the experience was consistent. You could tell the frame rate was lower but it was a smooth consistent experience. 

After using a g-sync monitor I think it would be difficult for me to live without.

Thank you for your detailed report.  When my now 6+ y/o Dell U3415W dies I will look for one--the problem is these screens generally never die 🤨

You can run GSync + Vsync + a frame limiter. For the longest time I only ran G-Sync (old FSX / P3D habits die hard...) - however Nvidia actually suggests running them combined. You don't get vsync back pressure and the frame pacing is more consistent.

I go for the best IQ I can get while keeping the sim smooth. If I cross the line and smoothness suffers, I back off my settings.

That's the way i run it.
