I am talking from experience and there are many videos and articles recommending it. Gsync works best with vsync enabled. I even linked to a source.

There are several conflicting reports on this, trust me I’ve read them all. 
nvidias latest driver has a built in frame capper, which eliminates the need for Vsync all together. (With G-Sync active)

It's actually pretty clear, you'll get minor tearing at the bottom of the screen with frame cap+vsync off. Nvidia having frame limiter doesn't change this fact. I won't say anything else, believe whatever you want.

As noted in the hotfix change log, the "new VRR option" pertains to monitors which have Variable Refresh Rate capability, making it irrelevant to those of us using monitors which are not.

I know, that's not what I asked at all. I have a G-Sync monitor, like many other people. We also know that this is mostly an automatic process of simply keeping it enabled and it works on whatever 3D application you use, if it's within the boundaries of the G-Sync specifications. However, what myself and I assume Ianrivaldosmith were wondering, what the tickbox does in addition to that, because it's usually not something you enabled in the 3D application itself. I have never seen an application or a videogame in all my years that has that.

So, I tested this using an 35 Inch Acer Z35P G-Sync (range available on this monitor is 1hz through 120hz) monitor at 120HZ  resolution 3440x1440. 

Variable refresh rate setting in P3Dv5 on. Vsync off in P3D and Frames set unlimited.

V sync forced on in NVIDIA control panel. (I also capped FPS to 60 here at the driver level, due to some flickering, but you don't have to.)

WOW, it actually works! This is the smoothest I have ever seen P3D perform, ever. Its on par with X-Plane Vulcan now! I am running almost locked at 60FPS stutter free over ORBX true earth GB. Fluctuates between 45-60 over cities, and as it is fluctuating, G-Sync does exactly what it is supposed to do. 

I am running pretty high settings too, System is i9900KS 5GHZ, RTX 2080ti, 32GB RAM. Also have a secondary monitor open for flight apps etc... 

Now, and only now, I can use P3D! Time to add some addons. I've got lots of headroom to play with. I also tested down to 30 fps by locking down there, and it was STILL smooth with this G-Sync setting. RESULT!

I agree, yours is the correct answer Ian - this is my experience so far with v5 HF1 as well, though I was running with fps target set at 100 because of my 100Hz monitor (I have the same resolution and monitor size as you).  I might try the unlimited frames based on your experience - that didn't work well with Gsync in 4.5, but it may very well be working properly now in v5.  The frame timing does look qualitatively more consistent - in previous versions I used to be able to see the little "surges" in fps even when the image was not stuttering, but those "speed ups" and "slow downs" are very rarely noticeable now.

I just did a flight where I started out at 70-80 fps, and flew somewhere more dense; when I opened up shift-Z at the end of the flight, I was shocked to see that my frames were in the low 40's... because it was no less smooth in that lower range than it was at double the fps.  Big improvement, because in v4 and earlier I could easily see when my frames were dipping down close to 40.  I agree, this a lot like the latest Xplane beta now.

The key settings for a Gsync monitor I think:

- Vsync and TB OFF in P3D (Vsync is at best redundant, and TB is unnecessary that is what VRR is for)

- VRR checked ON in P3D (THANK YOU LOCKHEED MARTIN FOR THIS FINALLY!!!)

- Gsync ON globally in Nvidia Control Panel for BOTH windowed and full screen (important becuase P3D is not a true full screen app, it is borderless windowed)

- Vsync ON but TB OFF in Nvidia Control Panel (to prevent tearing if you perchance exceed the refresh rate)

It was a long wait for L-M to finally figure out how to take advantage of variable-refresh technology, but it seems they have. Hallelujah!

I agree - I am curious what that tickbox is doing.  In most games, usually with Gsync we just make sure that Vsync is turned off in the game settings and then everything works. I'm not complaining though 😉

Yeah, it's amazing. I'm flying the PMDG 777 right now over Europe. I have a G-Sync Monitor and used the G-Sync Setting in the Driver all the years before. I "thought" it worked in the previous versions, because my Acer Monitor has a built-in refresh-rate display which seemed to be linked to the FPS in the past... it was still not smooth, though no tearing.

But now with P3D v5 HF1, I deleted all my P3D related Driver Settings, V-Sync OFF, Frames Unlimited and VRR to on. It's too good to be true, it's buttery smooth with this setting enabled, just wow 😮 I always wondered why P3D "felt" so stuttery in comparison to X-Plane, but this is completly gone now. I'm constantly smiling, because I enjoy it so much. Textures crisp and sharp even on Cruise Altitude, doesn't matter if it's near or in the distance. Full Scenery with ORBX (Global + OpenLC + FTX Region), FPS is range is 40-60FPS in the VC (40 is the lowest I've seen but even this doesn't matter anymore, but now that G-Sync actually works you don't have to care about it anymore), 70-110 FPS External View. CPU is an very old i7-2600k @ 4,5 GHz, GPU RTX 2070. TrueSky enabled, Ultra Cloud Texture Resolution.

Yes, due to the old CPU the FPS are fluctuating, but that's where the VRR Options really helps, you just don't notice anymore. Seems like locking FPS and messing around with FTFF is now obsolete with native VRR support. Feels like I upgraded my PC.

I never thought this would be possible, but I'm really happy with v5 now. Only ActiveSky and some official plane support is missing, otherwise.. Nice.:)

Yup, it’s that good I’m about to invest in some AI, ATC and A2A Comanche and continue my lockdown world tour from xplane and finish it in P3DV5! 
I’m actually pretty excited to have ATC (ProatcX) that will communicate with the AI around me! 
And the weather system, wow. 

Are you guys running full or windowed mode for P3D?

I am running in full screen, but of course it is really borderless windowed.  Beau from L-M did comment that you need to be running P3d with the full screen box checked in order to enable the flag in Windows that activates VRR (or something to that effect).

I have tested both locking frames at around 100 and also leaving the slider on Unlimited, and I think I prefer Unlimited.  Whether it actually is smoother at that setting or the same I am not sure, but the average fps seems to be higher with Unlimited setting which is a nice advantage.

I’m running unlimited in the sim. But using Nvidia control panel to lock them to 60. I seem to be having brightness flickering with the variable refresh rate if I don’t do that. It’s super smooth though. 
 

Oh, and yes, full screen mode, has to be enabled for it to work. I can still click over to my secondary monitor though.
