Remember this spreadsheet was built based on the Aerowinx 747 engine parameters and thrust logic therefore the derates are probably not as close as we might like. But it still works just great for giving you speeds and other information. Just remember one thing, which is to select an aircraft with the same MTOW as our PMDG (for example use LX-GVC as aircraft ID). Another thing I found is that you will not get a derate if you manually select a flaps 20 for takeoff. It will always end up telling you to use maximum thrust. You have to leave the flaps selection to auto and then press calculate to get a derate. The derate and the speeds will obviously be based on the flap selection that the spreadsheet chose. If you really want to takeoff with flaps 20 I suggest going through this process to find the derate and then entering flaps 20 and calculate again so that you'll find the adequate speeds for that flap setting. The obvious problem is that since derates are also flap-dependent you'll end up with an N1 figure that is not accurate. But until they find a way around this I'm happy using this spreadsheet.
