Steve I think it depends on the airport and the crew. My local airports only have short taxiways to the runways so if they started during taxi they would have to be very quick, so I think that's why most start during pushback. It then becomes to norm but I suppose if you had a long taxi and the crew was busy doing things during pushback then they might do it during taxi.
