A tiny meteorite or some tiny bit of space junk that is in orbit.  Might also be a manufacturing or assembly defect that finally made itself known.

They should make certain to have several rolls of the ubiquitous "speed tape, "gaffers tape" or "duct tape (aka: duck tape)" on board all space missions!

Luckily the leak appeared when they were already at the ISS. I don't want to think what would have happened if that happened during re-entry...

Actually they do have something like that. They carry patch kits. If a naughty micro-meteorite pokes a hole, they slap the kit on the inside. Pressure is inside of course so that's a huge help. For bigger holes they have to evacuate the section, Don pressure suits, then go in and patch the whole.

The Russians say they aren't going to bother fixing it. I guess it's not enough coolant loss to be an issue.

It's very good stuff. But not for Ducks. MythBusters made a boat out of it.
