As our technology gets more complicated and refresher training more limited due to extended and/or expanded operations these kinds of episodes are bound to happen. When I was a Marine during the Korean war we toted an M-1 Garand, a cartridge belt, a first aid kit and a canteen. I look at today's infantryman and I see a gaggle of equipment hanging from their shoulders, belts and helmets and I wonder if I could handle all that technology and equipment. The more options you have the more things can go wrong. It's incumbent on the manufacturers to exercise the ultimate in quality control when manufacturing these systems; on the best possible training for the operators of these weapons and systems; and on the rapid repairing of down systems including easy removal and replacement of a marginally operating system. It happened a lot in Viet Nam with over-zealous company and platoon leaders who volunteered their men for all of the dangerous missions.
