You know what, I think it might be OR73, and it's in Orbx PNW, where it's a flat surface. Check out the aerial photo on the AirNav page. Looks like it might be it.http://www.airnav.com/airport/OR73

I think it might be Madeira, which also has a runway on stilts.

I don't recall the exact location (it's been 10+ years since I saw that picture first), but as I fuzzily recall, it is a missionary strip somewhere in South America.The a/c land uphill, and native porters use ropes to pull the a/c the rest of the way to the top...

The telephoto lens used in this picture gives that impression, but I doubt it's much more than 20-30 degrees uphil

That can't be, because the Runway Information in the link you have posted says "Surface: asphalt", but the surface of the runway in the pix of the OP isn't asphalt at all. Wolfgang

Either that or try to stop the aircraft while landing downhill; stepping on the brakes in a short non-prepared runway... :(

If I recall correctly, I believe that it is/was flown to by the MAF (Missionary Aviation Fellowship) and I think it is in Africa.

Now that you mention it, I'm inclined towards Africa myself. I dimly remember reading about this jungle strip while researching information for Lionheart's Kodiak, which is -of course- primarily designed for missionary work...
