Doesn't the cabin crew and pilots have a code that allows them to enter from outside the cockpit? Why would he knock and slam on the door?

The  problem with that is one of the cabin crew could also be suicidal. There really is no way around this suicide problem

Could be  wrong  here, some one  can correct me here.   Cant  the pilot override  the door lock  from inside  so no one  can use  the door lock combination key  to get in?

Crew enter a keypad code and the crew in the cockpit open the door. There is a way to override this. And it is surprising that that wasn't done.

I am sure I read somewhere that there always has to be 2 people on the flight deck, so if a pilot goes for a break one of the cabin crew must wait in the flight deck until their return?

The pilot inside has to actively deny access - otherwise the pilot outside will eventually be able to enter.

Must admit that it is difficult to come up with a consistent theory other than one involving a deliberate action by the pilot remaining inside the cockpit.

Yes if so, that must have been horrifying. I've been on several flights where one of the pilots have stepped outside the cockpit just for a quick bathroom break, but this has always happened very quickly.

Why would the pilot knock on the door?  I assume that's just how its reported and the pilot was actually trying to follow the correct procedure in the video.

Thought the pilot  in the cockpit  can   deny this, anyway Ollie  has posted in the meantime

The thing is though, the pilot exiting only needs to leave the cockpit for a split second, the suicide guy then pushed the nose over, full throttle and boot in full rudder and aileron...there is no way anyone is coming back inside. What makes this unique is the pilot did a controlled descent. Why would you do that?

With Airbus envelope protection it is not that easy to bring down the plane.

You can crash an A320 while keeping it entirely within its "normal" flight envelope.

You can also revert from normal law to alternate and even to direct law, disabling all protections.

On the cockpit re-entry procedure, shouldn't there have been "beeps" on the CVR? Knocking on the door and even stronger actions have been reported, but no cockpit door "beeps"?

What happened to AVSIM

Maybe they need to introduce voice recognition for the door lock sensors.
