Every 2-3 months. A roadmap from July can be found at the bottom of this page: https://fselite.net/news/microsoft-flight-simulator-information-vr-steam-edition-third-party-info-and-future-plans/

I guess so far we've gotten Sim Update I and soon World Update I, so plans have probably changed a bit.

There is no but still.. about it. A scenery deveopler is not going to reprogram avionics. Simple.

As one who is on a somewhat slow internet connection, can I assume that one will not actually download this update unless one chooses to fly within Japan?

I would imagine that the landscape updates and poi's would be in the cloud, but any upgraded airports you would download, as it is currently.
