The true answer I think is somewhere in the middle.

We have definitely seen a nerfing of this product from its true potential, with LOD with trees and buildings definitely decreasing (or not being moddable up).  That's a deliberate decision by Asobo and we wonder why?  Could it be to level things down to the Xbox standard for that release, or to take a bit of bandwidth off the servers.

Terrain noticeably goes blockier and slightly less well defined at high altitudes no matter what the connection.  Is it terrible?  Not if everything is working, it still looks good.  But it's not super sharp, super crisp.  Some people won't even notice it, and some will feel like they're having their eyes gouged out.
 

But I think we are going to have to live with it.  We are purely at the whims of Asobo and how they seek to extract most money from the title and the industry.  It's about getting the best settings to work from you (I have my scaling on 150 for example).  This can take hours of tweaking, and it's frustrating, but it's the best we are getting for now.

We’ll see what happens when the XBox version has been released.

If stutters and lower resolution become worse it is a bandwidth issue.

If your bandwidth is enough you do not need a cache. Here I have 300 mb so I have them Off.

I doubt of the bandwidth used by the stream is that high, I think it is more about data caps for people that have them.

When looking at online games like Apex it does matter with how many users you are trying to play them.

Sometimes it is that crowded that the games are hanging, show pauses or low resolution terrain/objects.

Not to this extent, the area near ULMM is definitely buggy and surely it's not the only one. Look at the texture loading in this video, taken after a fresh install of Windows and MSFS at Ultra settings.

I opened a Zendesk ticket.

I see this too. I was landing at a glider club in the north east of England yesterday. And as I got around 100Ft, lots of planes started popping in to view! Immersion breaking.

Yep.. This totally looks like an internet issue. On a good connection if you slew 10 miles or so beyond the initial load point you can reproduce the same thing because you went out further so fast the game can't download and import the hi-res stuff fast enough.

A flight this slow showing the same thing. Yeah. Slow connection/server issues or something is tying up his CPU so bad it can't deal with it. Seeing his sig though, the latter is unlikely.
